HIPAA Notice
Notice of privacy practices under 45 CFR Parts 160 and 164
Our legal duty
Pasadena Clinical Group, doing business as PCG Health, is required by the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations at 45 CFR Parts 160 and 164 to maintain the privacy of protected health information, provide notice of our legal duties and privacy practices, and follow the terms of this notice as currently in effect.
How we may use and disclose protected health information
- for treatment, including coordination among clinicians or referring providers involved in your care
- for payment, including benefit verification, claims submission, utilization review, and billing support
- for health care operations, such as quality improvement, supervision, training, compliance review, and business management
- when required by law, public health reporting requirements, or court order as permitted or required by applicable law
- to avert a serious and imminent threat to health or safety when disclosure is permitted by law and professional standards
- for workers' compensation, law enforcement, or specialized government functions when authorized by law
Uses and disclosures that generally require authorization
Except as otherwise permitted or required by law, we will obtain your written authorization for uses and disclosures of psychotherapy notes, most marketing disclosures, and the sale of protected health information. You may revoke an authorization in writing except to the extent action has already been taken in reliance on it.
Your rights
- request restrictions on certain uses or disclosures, although we are not always required to agree
- request confidential communications by alternate means or at alternate locations
- inspect and obtain a copy of protected health information in the designated record set, subject to limited exceptions
- request amendment of records when you believe information is incomplete or incorrect
- receive an accounting of certain disclosures
- obtain a paper or electronic copy of this notice upon request
- be notified following a breach of unsecured protected health information when notification is required by law
Complaints
You may file a complaint with PCG Health or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. Changes to this notice
We reserve the right to revise this notice and make the revised notice effective for protected health information we already maintain and any information we receive in the future. The current notice will be available through our office and website.
